60 % Hematology 40 % Oncology Will not take any new Oncology patients during this period as they will be referred to our next closest location.
Patients Per Day:
12-15 Looking for minimum of 5 years of experience EMR:
Epic Scrub Color / Dress Code:
Business Casual Single Provider office, with no other in-office support for locum.
There are 2 physicians at sister location available for questions, case review, etc.
Required:

5 years of experience PA license is highly preferred Provider must complete Client Attestation form before presentation Client requires a new NPDB report to be submitted, at time of acceptance, before moving forward with credentialing.
Provider must provider permission to share NPDB in writing Must be fully vaccinated for Covid 19 Exemptions are only allowed for religious or medical reasons, once approved by the facility.
Current Flu Vaccine DEA must remain in state to maintain privileges.
